1. Please review the Student/Parent Handbook then complete this form by September 13, 2024. Please call your student’s campus if you have any questions. Thank you for your cooperation. 

As required by state law, the board of trustees has officially adopted the Student Handbook and Code of Conduct in order to promote a safe and orderly learning environment for every student. Our signatures below will certify that I and my student understand and consent to the responsibilities as outlined in the school district’s Student Handbook, Student Code of Conduct, and the Odyssey Academy Parental Involvement Policy and Compact as set forth in this Student/Parent Handbook.

2. Certain information about district students is considered directory information and will be released to anyone who follows the procedures for requesting the information unless the parent or guardian objects to the release of the directory information about the student. If you do not want Odyssey Academy to disclose directory information from your child’s education records without your prior written consent, you must notify the district in writing by checking the box below.

Odyssey Academy has designated the following information as directory information (for the limited purposes described above):

• Student’s Name
• Address
• Telephone Number
• School Attending
• Grade Level

Parents or adult students may object to the release of directory information by checking the box below.

3. Odyssey Academy uses a variety of resources to publicize district events and school news. Should you object to a photograph or other electronic image of your child on the school district website, newspaper, marketing brochures, publications, newsletter, or Odyssey Academy videos formally prepared for use both inside and outside the school district, please check the box below. By checking, you acknowledge that there may be times when your student is left out of class/group photos.

4. Parents and students are required to read Odyssey Academy’s Acceptable Use Procedures for the Network/Internet found in the Student/Parent Handbook. Odyssey Academy utilizes the internet for instructional resources at all grade levels. You and your child should discuss the Acceptable Use information and the consequences of misusing the equipment, the network, and/or internet access. Every effort is made to ensure the online safety of our students, but it is impossible for Odyssey Academy to filter all content available on the Internet. The District, its Officials, and Employees are released from all claims and damages of any nature arising from a child’s use of, or inability to use, the system. Odyssey Academy shall not be responsible for ensuring the accuracy, age appropriateness, or usability of any information found on the internet.

5. Odyssey Academy uses a variety of means to communicate with families about OA events and school news. Should you object to receiving automated calls or texts to your cellular device, please check the box below. By checking, you acknowledge that there may be times when your student is left out of class/school information and news.
